These were issued to all pupils last week.  Can you please check, amend, add and delete any details as necessary paying particular attention to mobile numbers and e-mail addresses.  Please ensure we have a minimum to two adults that we can contact in case of illness/emergency.

It is important that the sections headed Ethnicity and Home Language are completed.  If you do not wish your child’s ethnicity to be recorded please leave the box blank.  Can you please return both these forms via your child’s classteacher no later than Friday 19 October.  Thank you.
